We collect your information for the purpose of providing healthcare services tailored to your specific needs.
Pertinent information will be shared with relevant parties in order to assist with the outcome of the intervention. Prior to sharing the information, Vital Therapy Services will advise you on who will be the recipient of that information.
At our initial visit, we will discuss how Vital Therapy Services will manage your personal information and we will ask you to read and sign our Consent Form. The consent you give us to share your personal information will cease as soon as the therapy intervention ceases.
Wherever possible, Vital Therapy Services uses information in a de-identified form. All personal information collected from individuals will be securely stored by Vital Therapy Services and protected from unauthorised use or access.
